The Problem

You’re about to leave the country with a receipt and a vague memory of the doctor’s name — and the insurance company is going to ask for 9 documents you didn’t know you needed.

😰

You got the hospital bill receipt — a single page showing the total in the local currency — and assumed that was enough for the insurance claim. It’s not. The insurer needs an itemized breakdown with procedure descriptions

A receipt showing ‘$3,800 — Hospital General’ tells the insurer nothing about what you were treated for, what procedures were performed, or whether the charges are reasonable. Insurance companies require an itemized bill: each consultation, each test, each medication, each night of stay listed separately with descriptions and ideally procedure codes. Without it, they can’t verify the charges and the claim is flagged for additional documentation — which you now need to request from a hospital in another country via email that goes unanswered.

💸

The insurer denied your $6,000 claim because the medical report was in Spanish with no English translation — and their policy requires all supporting documents in English

Most US-based travel insurers require documentation in English. If your treatment was in a non-English-speaking country, the medical report, diagnosis, and discharge summary need to be translated. Some hospitals will provide an English version on request (private hospitals in tourist areas often do this routinely). Others require you to arrange a certified translation. Getting a document translated AFTER you leave costs $50–$150 and takes weeks. Asking the hospital for an English version BEFORE you leave takes 5 minutes.

😵

You saw three providers — the ER doctor, the radiologist for the X-ray, and the pharmacy — and you have receipts from only one of them because you didn’t realize each bills separately

Overseas hospitals sometimes bill like US hospitals: each department or provider generates a separate charge. The ER physician, the imaging department, the lab, the pharmacy, and the hospital facility may all issue separate invoices. Collecting receipts from only the main billing desk leaves the other charges undocumented. The checklist ensures you get a receipt from EVERY provider involved in your care — because the insurer will only reimburse charges you can prove with documentation.

🤷

You left the hospital 3 weeks ago and now the insurance form asks for the ‘attending physician’s name and medical license number’ — and you have no idea who treated you or how to contact the hospital from 8,000 miles away

Insurance claim forms ask for specific details that seem easy to provide at the time but become impossible to recall weeks later: the doctor’s full name, license or registration number, the hospital’s address and phone, the date and time of admission. The checklist includes these details as items to record DURING the hospital visit — before the stress fades and the details blur. A 2-minute note on your phone at the hospital saves hours of frustrated calls later.

What You Get

Not ‘keep your receipts.’ — The complete, treatment-specific list of every document your insurer will demand — collected before you fly home.

Questions

Everything you need to know.

What’s the most commonly missing document?

The itemized bill. Most patients receive a summary receipt (total amount only) and assume it’s sufficient. The insurer requires a line-by-line breakdown: each service, each test, each medication, each day of stay — with descriptions and ideally procedure codes. Request the ‘itemized invoice’ or ‘detailed statement’ from the billing department before you leave. If the hospital only provides a summary, ask specifically for the full breakdown — most can generate one on request.

Do I need documents translated?

Most US-based insurers require documents in English. Many private hospitals in tourist areas provide English-language reports routinely — just ask. If the hospital only provides documents in the local language, request a certified translation. Getting this done in-country is usually cheaper and faster ($20–$50) than arranging it from the US ($100–$200 and 2–4 weeks). Ask the hospital if they offer translation services or can recommend a local translator.

Can I get documents after I leave the country?

Technically yes, practically difficult. Overseas hospitals are not required to respond to requests from foreign patients. Email requests may go unanswered for weeks or months. Phone calls across time zones and language barriers are frustrating. Some hospitals charge fees for records requests. The success rate for remote document retrieval varies by country: high in Western Europe and Southeast Asian private hospitals, low elsewhere. The best strategy is always to collect everything BEFORE you leave.

What if my treatment involved multiple hospitals or doctors?

Collect documents from EACH provider separately. If you were treated in the ER, then transferred to another hospital, then saw a specialist — each facility generates separate records and separate bills. The claim needs documentation from all of them. The Complex Treatment Organizer helps you build a master list linking every provider to every charge to every document, plus a chronological narrative that ties the entire treatment episode together for the insurer.

How long do I have to file the claim?

Deadlines vary: most travel insurance policies require filing within 60–90 days of treatment. US health plans with international coverage typically allow 90–180 days. Credit card travel benefits vary by issuer — some require notification within 20 days of the incident. Filing late can result in automatic denial regardless of documentation quality. The checklist includes filing deadlines for your specific insurance type. General advice: file as soon as you’re home with complete documents rather than waiting.
